By Too short, I mean a few comments would be helpful. I am not sure what are actual values and what should be plugged in with something else. For instance, the radio button definitions: are the values Page1, 2 and 3 the actual values or do I plug in the page names that I want the form to go to after sumbit? Are those values supposed to correspond exactly with the name parameter in the Form definition? Also, I am confused about the function name (go(what)) and the onClick value being go(this.form). How do they relate? How does the function get called? Any further explanation would be helpful. This would be very handy for me if I could make it work.

thanks for the example...I was able to make the action property different based on what checkbox was checked. How can I make the action open in the same window instead of opening another IE window?
